Premier at Home

To ensure you receive the highest level of comfort in care, your Premier membership includes three at-home visits per year, thoughtfully coordinated by your dedicated Premier primary care physician and delivered by a trusted vendor based on your needs. These visits offer the same exceptional, personalized care you enjoy at The Health Center, all within the privacy and comfort of your home.

Should you prefer your Premier provider to deliver care at home, we are happy to arrange in-home visits for an additional fee.

 

Are home visits included in my annual Premier Membership?

Yes, your annual Premier membership includes three at-home visits per year. We partner with variety of vendors for these at-home visits depending on your need. Your dedicated Premier primary care physician will coordinate the visit and ensure that the clinician is completing the appropriate tests.

Additional at-home visits beyond the three included in your annual Premier membership are available at an additional fee. Depending on your specific care needs and circumstances, your Premier physician may be able to conduct these additional at-home visits. 

Can I see a Premier provider for my three at-home visits included in my Premier membership?

Our trusted vendor partners perform the three included at-home visits. However, if you prefer in-person care from your Premier physician, at-home visits can be arranged for an additional fee of $1,200 per visit. Please note that certain tests and procedures (e.g., blood draws and imaging) are not included in the Premier provider at-home visit. These will be coordinated through a vendor and billed separately.

How do I know if I can see my Premier physician for my at-home visit needs?

With approval from your Premier physician, we can accommodate at-home visits under the following conditions:

  • You are located in Manhattan, AND
  • You are requesting an at-home visit for a non-urgent need (with an earliest response time of 2 business days), between 8 am–7 pm Monday through Thursday, and 9 am–5 pm on Fridays, AND
  • Your at-home visit requires any of the following:

Included Services:

  • Basic medical evaluation
  • COVID-19 testing
  • Vaccines (available onsite at HCHY, except RSV)
  • IM/subcutaneous injections (antibiotics, biologics, e.g., Prolia)
  • Nebulizer treatments (asthma, cystic fibrosis, COPD, etc.)
  • Urine sample collection (urinalysis & urine drug screen)
  • Pregnancy test
  • Flu test
  • Strep test

Add-On Services Available Fee-for-Service:

  • Imaging
  • EKG
  • Ultrasound
  • X-ray
  • Blood draw/labs
  • IV therapy
  • Foley catheter insertion/change

How do I schedule a Premier physician at-home visit?

To request and schedule an at-home visit, members may contact a Premier Health Navigator by:

When requesting a Premier provider at-home visit, please specify your at-home visit need/reason and that you wish to see your Premier provider. Your request will be reviewed by the Premier clinical team, who may reach out as needed to collect any additional clinical information needed. Once the request is approved, your Premier Health Navigator will schedule your Premier physician at-home visit and walk you through the next steps. You will receive an appointment reminder 1-2 business days before your scheduled at-home visit.

What forms do I need to complete before my Premier at-home visit?

You will be required to complete two forms through MyChart check-in. Completing these forms is mandatory to confirm your Premier at-home visit.

Is my dedicated Premier physician also my at-home visit provider?

We will make every effort to arrange your at-home visit with your dedicated Premier physician.

If your dedicated Premier physician is unavailable, they will work with another Premier provider to ensure all your at-home care needs are met.

How long is a Premier PCP at-home visit?

Typically, an at-home visit will last 60 minutes. Please note that at the provider’s discretion, the home visit can be discontinued at any time if any safety concerns arise.

What if I need to cancel a Premier physician at-home visit?

If you need to cancel your visit, please provide at least a 24-hour notice for cancellation.

How much does it cost for a Premier physician to conduct an at-home visit?

All at-home visits conducted by a Premier physician will incur an additional fee of $1,200 per visit.

When will payment be collected for a Premier physician at-home visit?

Payment will be collected at the time of scheduling your at-home visit.

Can insurance be used to pay for a Premier physician at-home visit?

Insurance cannot be used to pay for the Premier physician at-home visit, but a receipt can be provided upon visit completion if you would like to submit it to insurance.
